Measures for the implementation of the state program of the Russian Federation "Accessible Environment" in medical organizations providing primary health care to adults and children

In medical organizations of the state healthcare system, primary health care for adults and children, including the disabled, children with disabilities, people with limited mobility is provided in full, provided for by the Territorial Program of State Guarantees of Free Provision of Medical Care to Citizens in the City of Moscow, dispensary activities are carried out, including comprehensive medical examinations at home with the participation of specialist doctors and the use of available diagnostic capabilities. In medical organizations, departments of medical care at home have been established, one of the areas of which is the provision of medical care, including to non-transportable patients or those with impaired motor functions. All those in need (handicapped and lonely) are provided with targeted assistance. According to the indications, taking into account the existing diseases, the availability of an individual rehabilitation and habilitation program (IPRA), they undergo the necessary rehabilitation measures.

The share of buildings of medical organizations accessible to the disabled and other people with limited mobility is increasing (73.7 % as of 01/01/2020), in 2019 the increase was 4.8 %.

In each administrative district of Moscow, there are medical organizations that provide primary health care to the population (hereinafter referred to as medical organizations), equipped with diagnostic x-ray equipment with swivel tubes, transforming tables with a mechanism for raising and lowering the surface, allowing examinations of people with limited mobility. To ensure the availability of dental care for people with severe disabilities in accordance with the Decree of the Government of Moscow dated February 17, 2009 No. 115-PP “On the Year of Equal Opportunities in the City of Moscow for the period until 2020”, dental offices have been organized in accessible places (mainly for 1 floors of buildings), dentists-therapists and orthopedists provide treatment and prosthetics at home for people with disabilities who have difficulty with independent movement.

In medical organizations, together with public organizations of the disabled of the city of Moscow, in accordance with the "Regulations on conducting a survey of facilities of medical organizations of the state healthcare system of the city of Moscow, in terms of creating a barrier-free environment for people with disabilities and other low-mobility groups of the population", measures are being taken to create a barrier-free environment for persons with disabilities:

— access of persons with disabilities to the place of provision of services in remote mode, as well as at the place of residence;

- allocation of parking lots for disabled vehicles;

— paving with tactile paving slabs with cone-shaped reefs for blind patients;

- the presence in the medical organization of removable wheelchairs and the sufficient width of doorways, flights of stairs, platforms for their movement;

- painting the first and last steps of flights of stairs in yellow;

- the presence of reed holders;

– the operation of elevators adapted for the disabled (light, sound and / or Braille dotted displays);

- inscriptions, signs and other textual and graphic information - in Braille and / or on a contrasting background;

- equipment of buildings with handrails and ramps, accessible entrance groups and sanitary and hygienic premises;

— equipment of objects with a vertical lifting platform (ramp).

As part of the overhaul of buildings of medical organizations, it is planned to form an accessible barrier-free environment at the stage of developing design and estimate documentation.

The official websites and information stands of medical organizations contain information aimed at informing the disabled community and tolerant attitude towards the disabled.

On the basis of the order of the Ministry of Health of Russia dated December 29, 2012 No. 1705n “On the procedure for organizing medical rehabilitation”, medical rehabilitation is carried out, including 3 stages. The third stage is carried out in the departments of medical rehabilitation of polyclinics, visiting teams at home, sanatorium organizations.

As of 01/01/2020, within the framework of the Territorial program of state guarantees for the provision of free medical care to citizens in the city of Moscow, medical rehabilitation services at stage 3 are provided in 59 medical organizations. In 26 medical organizations there are departments of medical rehabilitation (hereinafter - MMR), in 20 - physiotherapy departments (hereinafter - PTO), in 14 - separate rooms that are not part of MMR and PTO, all services are provided at the expense of MHI.

The departments listed below are:

— 188 physiotherapy rooms;

— 102 halls of physiotherapy exercises (including mechanotherapy);

— 7 manual therapy rooms;

— 145 massage rooms;

– 15 reflexology rooms.

In 46 medical organizations there is an opportunity to receive medical rehabilitation services in combination: physiotherapy, physiotherapy exercises, massage.

As of 01/01/2020, 926,992 disabled people are observed in medical organizations, of which 832,560 people received rehabilitation services in 2019 (89.9%). Preventive courses of treatment in a day hospital in 59 medical organizations in 2019 were received by 34,022 people with disabilities.

In order to implement the IPR activities in 2019, 599,284 people applied to medical organizations, rehabilitation and / or habilitation measures were taken to implement the IPR for 586,549 disabled people (97, 9%).
