About the rules and terms of hospitalization
The main indications for referral of a patient from a dental clinic to medical and diagnostic specialized units of inpatient institutions (consultative and diagnostic departments and centers that are structural units of inpatient medical and preventive institutions, as well as dispensaries and scientific and practical centers that provide advisory and medical and diagnostic assistance in on an outpatient basis at the prehospital stage and after discharge of patients from the hospital) are:
- the need for planned hospitalization to receive specialized, including high-tech, medical care.
- - lack of effect from the ongoing treatment (aftercare), the need to correct the ongoing therapy or individual selection of drugs in stationary conditions (including day hospital, hospital at home);
- - the development of complications in patients undergoing aftercare;
- - lack of an appropriate specialist, type or possibility of examination that the patient needs;
- - the need to make a coordinated decision on hospitalization for the provision of specialized, including high-tech, medical care;
- - the patient has a diagnosis that requires the provision of specialized, including high-tech, medical care, which can be carried out on an outpatient basis or in a day hospital of a specialized treatment and diagnostic unit of an inpatient institution.
The procedure for referring patients from the clinic to a specialized center:
- – referral of a patient to a specialized center is carried out in accordance with the doctor’s appointment schedule:
- - in the registry of a specialized center, a separate telephone number is allocated and a responsible person is appointed to keep records of patients for a consultation or planned hospitalization.
Medical workers, if it is necessary to refer the patient to a specialized center, agree on the date and time of admission by phone, the patient is given an extract from the medical record of the dental patient (form 027 / y) and a signed referral form (form 057 / y-04).
A patient referred to a specialized center must provide:
- – an identity document (passport);
- compulsory medical insurance policy;
- - an extract from the outpatient card (form 027 / y);
- - referral (form 057 / y-04), filled out by the attending physician.
The waiting period for an agreed appointment at a specialized center should not exceed 7 days.
The period of examination for planned hospitalization should not exceed 10 days.
The waiting period for planned hospitalization should be no more than 10 days for patients with a therapeutic profile and no more than 14 days for patients with a surgical profile, with the exception of special cases:
- the need to receive high-tech medical care;
- - the desire of the patient to be treated by a particular doctor;
- – carrying out repair work in specific structural units of the hospital, etc.
When a medical specialist diagnoses a patient with an emergency condition, characterized by a sudden acute illness, condition, exacerbation of a chronic disease without obvious signs of a threat to life, hospitalization is organized through an ambulance station in accordance with the order of the Ministry of Health of the Russian Federation dated June 20, 2013 No. 388n “On approval of the procedure for providing emergency, including specialized, medical care.”
If the patient refuses to be admitted to hospital, a proper refusal is issued, indicating the possible consequences, with an appropriate entry in the medical documentation and the signature of the patient or his legal representative.
Upon the provision of consultative and diagnostic assistance or diagnostic studies (an additional examination to clarify the diagnosis or prepare for hospitalization), the patient is issued and issued a medical report with recommendations for further treatment and examination.
Upon completion of inpatient treatment, the patient is issued and given a detailed extract from the medical history with recommendations for further treatment and examination, at the same time, information about the treatment performed and recommendations for aftercare are sent to primary health care institutions at the place of actual residence of the patient.
The referral of patients for post-hospital follow-up care, rehabilitation treatment or dynamic observation from specialized centers to city polyclinics occurs in the same manner as described above. The procedure also provides for the active participation in the aftercare of medical specialists of the hospital and the specialized center where the patient was treated.
