For citizens of the Russian Federation aged fourteen and over:

• a passport of a citizen of the Russian Federation or a temporary identity card of a citizen of the Russian Federation, issued for the period of issuing a passport;
• compulsory health insurance policy.

For persons entitled to medical care in accordance with the Federal Law "On Refugees":

• a refugee certificate or a certificate of consideration of the application for recognition as a refugee on the merits, or a copy of the complaint against the decision to deprive the refugee status filed with the FMS of Russia with a mark on its acceptance for consideration, or a certificate of temporary asylum on the territory of the Russian Federation;
• compulsory health insurance policy.

For foreign citizens permanently residing in the Russian Federation:

• passport of a foreign citizen or other document established by federal law or recognized in accordance with an international treaty of the Russian Federation as a document proving the identity of a foreign citizen;
• resident card;
• compulsory health insurance policy.

For stateless persons permanently residing in the Russian Federation:

• a document recognized in accordance with an international treaty of the Russian Federation as a document proving the identity of a stateless person;
  resident card;
• compulsory health insurance policy.

For foreign citizens temporarily residing in the Russian Federation:

• passport of a foreign citizen or other document established by federal law or recognized in accordance with an international treaty of the Russian Federation as a document proving the identity of a foreign citizen, with a mark on a temporary residence permit in the Russian Federation;
• compulsory health insurance policy.

For stateless persons temporarily residing in the Russian Federation:

• a document recognized in accordance with an international treaty of the Russian Federation as a document proving the identity of a stateless person, with a mark on a temporary residence permit in the Russian Federation or a document of the established form issued in the Russian Federation to a stateless person who does not have a document proving his identity ;
• compulsory health insurance policy.

For a representative of a citizen, including a legal one:

• an identity document and a document confirming the authority of the representative;

In case of change of residence:

• document confirming the fact of change of residence.

In case of change of surname (first name, patronymic):

• a document confirming the fact of changing the surname (first name, patronymic);
• compulsory medical insurance policy for a changed surname (first name, patronymic).

The procedure for attaching citizens to a medical organization

• Order of the Ministry of Health and Social Development of the Russian Federation dated April 26, 2012 No. 406n “On approval of the procedure for choosing a medical organization by a citizen when providing him with medical care under the program of state guarantees of free provision of medical care to citizens”
• Order of the Department of Health of the City of Moscow and MGFOMS dated April 27, 2015 No. 335/142 “On approval of the regulations for attaching and registering citizens insured under compulsory medical insurance to medical organizations of the state healthcare system of the city of Moscow providing primary health care and included in the register of medical organizations operating in the field of compulsory medical insurance in Moscow using EMIAS"
